G.A. Carmichael Family Health Center proudly announces that all five of its locations are recognized by the National Committee for Quality Assurance (NCQA) as Patient-Centered Medical Homes (PCMH).
NCQA Patient-Centered Medical Home Recognition is a nationally recognized program that evaluates healthcare practices on their ability to deliver coordinated, patient-centered care using evidence-based standards focused on quality improvement, access, and team-based care. This achievement reflects months of collaboration, documentation refinement, workflow alignment, and dedicated effort from clinical and administrative teams across all five G.A. Carmichael locations.
This milestone was the result of a coordinated organizational effort led by Chief Information Officer Tyrone Thigpen and Chief Medical Officer Dr. Yvette McLaurin. Together, they guided the alignment of documentation, strengthened clinical and operational standards, ensured regulatory compliance, and established consistency across all five locations.
“PCMH Recognition reflects how our entire organization works together with excellence, accountability, and compassion,” said Dr. James L. Coleman, Jr., Chief Executive Officer. “This achievement affirms the collective effort of our teams to deliver coordinated, patient-centered care that strengthens the health of our communities.”
PCMH Recognition affirms that G.A. Carmichael’s care model emphasizes coordinated, team-based collaboration, improved access to care, proactive follow-up, population health management, and continuous quality improvement. With sites in Canton, Yazoo City, Greenwood, Belzoni, and Carthage, G.A. Carmichael Family Health Center remains committed to delivering organized, comprehensive, patient centered care aligned with nationally recognized standards.
